OUTBOUND FORMS and DOCUMENTS

Long Term Application	Comprehensive applications for US Students applying to participate in Long Term (Academic Year) program. Please read the instruction page carefully. This form need to be completed in Quadruplicate. It is best to complete the application and print out four copies before you get the required signatures. Original signatures are require on all four copies of the application.
Long Term Supplemental Application	This form must be completed and submitted to the sponsoring Rotary Club when submitting the long term application
Country Seletion List	Use as a guide. You will be given one to complete at the District Interviews
